Our Prairie Crossing campus is located at 1280 Pine Haven Ln, Sheboygan Falls, just south of Highway 23 and west of Highway 32.

Pine Haven takes great pride in the food we serve to our residents. Meals are served in our dining rooms which provide a restaurant-style setting and meal times are flexible.
Some of our programming includes: brain games, offsite trips, musical entertainment, bible studies, hymn sings, baking activities, crafts, and of course bingo.
Prairie Crossing is licensed as a Residential Care Apartment Complex (RCAC), allowing residents to live independent lives, secure in the knowledge that help is on-site, should it be needed.
Each private apartment includes a private bathroom with shower, refrigerator/freezer, microwave, stove with oven, washer/dryer, and dishwasher. Electricity, heat, water, sewer, air-conditioning, telephone, basic cable, light housekeeping, and a meal plan of 3-meals per day are included. Our Independent Living and Supported Independent Living apartments come in four sizes:
