Our specially designed memory care households specifically address the needs of individuals who have problems with memory, language, or thinking. Our households were named Covenant Home to remember God’s Covenant with His people through the gift of Jesus and His faithfulness to those who accept and follow Christ. This Covenant is at the heart of our mission and vision, which drives us to provide our residents’ safety, security, and the comforts of home.
Memory Care is a specialized level of care provided to seniors who have Alzheimer’s and other forms of dementia. Our memory care communities provide secure environments where our staff can closely monitor our resident’s physical, emotional, and spiritual health.
Pine Haven Christian Communities, voted Best of Sheboygan County in Memory Care for 2024, is the largest dedicated memory care provider in the county. Our levels of memory care help our residents and families manage costs while providing customized care to meet physical and mental needs. It is a unique solution that we believe will help provide better, more compassionate care to our residents.
We offer three different memory care programs, early to mid-stage (offered at our Giddings Ave Campus), mid to late-stage (offered at our Haven Drive Campus) and a high acuity memory care program (offered at our Haven Drive Campus). Residents will get priority placement to move through the continuum should their needs change.
Staff specially trained in dementia care - All staff are required to complete a series of Teepa Snow dementia care programming and participate in a “dementia virtual tour” to better understand their perspective and provide compassionate care.
Looping hallways and enclosed courtyards - Looping hallways so residents who wander never come to the end of a path. These environments help to alleviate anxiety.
Resident-centered care - Creating a schedule while allowing flexibility around the residents' needs and desires, helping to reduce stress.
Quick Admissions Process - Our streamlined admission process allows for rapid placement when urgent care is needed, ensuring timely support for your loved one.
Respite Care - We offer respite services in our memory care program, providing temporary relief for caregivers while ensuring high-quality care for your loved one.
Regular hymn songs, church services, and devotions - Many residents find these activities soothing and familiar. Residents who may have difficulty communicating through words can still remember and sing along to their favorite hymns.
Directed activity programming - Our Life Enrichment programming is designed to keep residents engaged through mind fitness exercises and cognitive training.
Our Haven Drive campus is located at 220 Haven Dr., Sheboygan Falls, just south of Highway 23 and west of Highway 32.
Our Giddings Avenue campus is located at 531 Giddings Avenue in Sheboygan Falls, just north of Highway 28 on Highway 32.
