To ensure we continue meeting the needs of the seniors and serving the community, Pine Haven transitioned the nursing home to be an assisted living environment that benefits residents with high care needs, such as they were in the skilled nursing program.
Q. What is assisted living?
A. Seniors with assisted living services are provided with extra personalized care while living in a residential setting. Pine Haven's multiple levels of Assisted Living Programs are at our Oostburg campus and two Sheboygan Falls campuses, Haven Drive and Giddings Avenue. Our Giddings and Haven Drive campuses provide low to high-acuity care and support, while our Oostburg Campus can provide low to mid-acuity care and support. Residents may move within our continuum of care as individuals' needs exceed the level of care offered in a program.
Q. What does higher acuity mean?
A. Whether it's assistance with a mechanical lift or one-to-two staff member transfer and mobility, we are here to help. We will also be able to meet the needs for more extensive assistance with activities of daily living (ADLs). Examples include Foley catheter care, tube feedings, and wound treatment and management.
Q. How is assisted living different than a nursing home?
A. We are dedicated to providing the highest level of care for each of our residents with RN oversight, and we will continue to do so with compassion and understanding through this new program. Our expanded assisted living program allows Pine Haven to meet the residents' preference to age in place by staying in a less restrictive assisted living setting. Staff will continue to be trained to care for residents with complicated diagnoses, required medical management, and other individual needs. Christ-centered activities and quality of life will remain the same, including housekeeping, food service, life enrichment, activities, and more.
Q. Can residents receive therapy while living at Pine Haven?
A. Yes. Residents can still receive therapy under Medicare Part B benefits. Our goal is to provide the care and services you need at the level you need while helping you maintain independence. We are pleased to offer physical, occupational, and speech therapy to new and current residents across all levels of care. This includes residents within our expanded assisted living. We customize therapy to your unique needs, using proper assessments to help you achieve your goals.
Q. Does Pine Haven still offer a continuum of care?
A. Yes. Our resident-centered model of care for assisted living services is tailored to meet your individual needs. We are excited to expand into more levels of assisted living care and services for new and existing residents. Pine Haven's continuing care offers an evolved assisted living approach designed to help you maintain your independence while providing the level of care you need.
Q. What if my loved one needs skilled nursing care?
A. Pine Haven is committed to working with residents and their families as care needs increase. To ensure resident needs are met, we work well with various Skilled Nursing Homes in the neighboring communities when the only option for your loved one is a skilled nursing facility.
