This full-time position will be based out of our Haven Drive campus and will have the following responsibilities:

  • Develop and strengthen the dementia care program at Pine Haven Christian Communities, including direct oversight of the dementia care program at Haven Drive and being a resource for the dementia care program at Giddings Avenue campus
  • Provide direct oversight of Covenant, Faith, and Fellowship Homes on Haven Drive campus
  • Lead and manage staff within the CBRF assisted living programs on the Haven Drive campus
  • Participate in quality improvement initiatives
  • Be responsible for overall care needs of residents:
    • Facilitate resident and family meetings, including Individual Service Plan (ISP) meetings
    • Complete or delegate as appropriate and review care of residents:
      • Assess potential residents and recommend appropriate placement based on resident needs
      • Complete admission paperwork
      • Medication and treatment administration
      • Create and maintain ISP, including changes
      • Communication with families regarding clinical needs/concerns of residents or changes of condition
      • Communication with physicians, hospital staff, clinical and social services staff of other PHCC facilities regarding clinical needs of residents
      • Monitor infection control risks

Qualifications:

  • Current WI RN license or bachelors degree in a health care-related field required
  • Have a passion for serving individuals with dementia
  • Certified Dementia Practitioner (CDP) or Ceterfied Dementia Care Manager (CDCM) certification or willingness to earn this within one year
  • Prior experience in long-term care required, preferably CBRF setting
  • Prior experience in a supervision or management role preferred
  • Strong teamwork and communication skills are critical
